A群鏈球菌 （group A streptococcus）

A群鏈球菌（group A streptococcus），又名化膿性鏈球菌（Streptococcus pyogenes），是屬於革蘭氏陽性完全溶血鏈球菌。A群鏈球菌是人類的主要致病菌之一。 感染主要引起咽喉炎、扁桃腺炎、中耳炎、鼻竇炎等，同時也可能引發嚴重性疾病，如猩 紅熱、毒性休克症候群、菌血症、壞死性筋膜炎等，有時也會引起非化膿性發炎併發症如 急性風濕熱和急性腎絲球腎炎。依據WHO統計，每年至少有181,000,000人感染A群鏈球菌， 每年至少有517000人死於A群鏈球菌嚴重型感染，其嚴重性更是在全球感染微生物致死排 行上名列第九，足可見其嚴重性。

雖然A群鏈球菌是一株古老的細菌，但在近十幾年來，A群鏈球菌所引起的疾 病嚴重性卻大大增強，病人常於入院後48小時便死亡。為什麼這一株菌會突然增強它的毒 性是所有科學家想知道的謎，而我們實驗室從民國82年開始即參與此研究，主要針對A群鏈 球菌流行病學、抗藥機轉、毒力因子之致病機轉與調控方式，以及細菌對抗氧化壓力之機轉 來研究。

B群鏈球菌（group B streptococcus）

B群鏈球菌，也稱為Streptococcus agalactiae，是屬於革蘭氏陽性完全溶血 鏈球菌。B群鏈球菌最常造成新生兒敗血症及腦膜炎，也是引起懷孕婦女重要傳染疾病的 病原菌之一。實驗室目前主要針對B群鏈球菌吸附至細胞的可能機制作研究，以及環境因 子對生物膜形成能力之影響來探討。

幽門桿菌（Helicobacter pylori）

幽門桿菌是革蘭氏陰性菌，且在1983年才由Marshell與Warren發現，兩人也 於2005年獲得諾貝爾生理醫學獎。幽門桿菌感染會引起從輕微的胃不適，到胃潰瘍、十二 指腸潰瘍，甚至是致死性的胃癌，而其盛行率在已開發國家可達二成至五成，在開發中國 家甚至會大於八成，足可見幽門桿菌對於人類健康的威脅性。

目前實驗室主要探討幽門桿菌與宿主之間的互相作用，包含細菌吸附至胃 組織的機制，以及吸附因子的調控機轉。在宿主方面，我們也結合臨床資源，分析宿主的基 因變異性與疾病嚴重度的關連性；此外，實驗室也試圖開發具抑制幽門桿菌能力的益生菌，來 提升幽門桿菌的治癒率。

大腸桿菌（Escherichia coli）

大腸桿菌是革蘭氏陰性桿菌，常見於溫體動物的大腸內。雖然大腸桿菌屬 於人類正常菌叢，但它亦可以侵入人體組織引起相當多樣性的疾病，包含急性腸胃炎、泌尿 道感染、腦膜炎等。目前實驗室主要探討引起泌尿道感染的大腸桿菌，並著重在大腸桿菌的 吸附因子上。同時我們也分析其他引起泌尿道感染的細菌，如克雷白氏肺炎菌（Klebsiella pneumoniae）之致病機轉。

細菌抗藥機制

目前實驗室主要針對革蘭氏陰性細菌對β-lactam, quinolone藥物的抗藥機轉以及革蘭 氏陽性球菌對Marcolides之抗性，進一步深入探討其機制。
